Adult Dance Class Participation Agreement, Assumption of Risk, and Release: Please read this agreement carefully before participating in any class taught by Gwynenn Taylor Jones at Black Dirt Dance. 1. Voluntary Participation: I voluntarily choose to participate in adult ballet, pointe, pre-pointe, conditioning, stretching, and related movement activities taught by Gwynenn Taylor Jones. I understand that participation is optional and that I may decline or stop any movement or exercise at any time. 2. Acknowledgment of Physical Activity and Risks: I understand that dance instruction involves physical exertion and may include, among other activities, stretching, balancing, turning, jumping, repetitive movement, floor work, pointe or pre-pointe exercises, and strength and conditioning work. I understand that participation carries inherent risks, including but not limited to: Slips, trips, and falls; Muscle soreness, strains, or sprains; Joint, tendon, ligament, or bone injuries; Aggravation of a previous or existing condition; Dizziness, fatigue, or loss of balance; Injury arising from pointe shoes, dance flooring, barres, equipment, or interaction with other participants; Other injuries that may occur during physical activity. I knowingly and voluntarily accept the inherent risks associated with participating in these activities. 3. Health and Ability to Participate: I represent that I am physically able to participate in the class or have received appropriate medical guidance regarding my participation. I agree to inform Gwynenn Taylor Jones of any injury, limitation, pregnancy, medical concern, or other condition that may affect my safe participation. I understand that I am not required to disclose detailed medical information, but I should communicate any information reasonably necessary for the instructor to offer appropriate modifications. I understand that Gwynenn Taylor Jones is not a physician, physical therapist, or medical provider and that dance instruction is not medical advice, diagnosis, rehabilitation, or treatment. 4. Personal Responsibility: I agree to: Participate within my own abilities; Follow reasonable class and safety instructions; Use appropriate footwear and equipment; Stop participating if I experience pain, dizziness, shortness of breath, or other concerning symptoms; Request a modification when needed; Avoid attempting movements that I do not feel prepared to perform safely. I understand that modifications may be offered but cannot eliminate every risk of injury. 5. Pointe and Pre-Pointe Participation: I understand that registration for and participation in the preceding Open Adult Ballet class is required in order to participate in the Pointe or Pre-Pointe portion of class. I understand that participation en pointe is subject to the instructor’s assessment and discretion. Gwynenn Taylor Jones may require me to modify exercises, work at the barre, participate on flat, or complete pre-pointe strength and preparation exercises instead of dancing en pointe. I agree to follow all pointe and pre-pointe safety directions. 6. Assumption of Risk and Release: To the fullest extent permitted by New York law, I voluntarily assume the inherent risks associated with participating in these dance and movement activities. To the fullest extent permitted by law, I release and hold harmless Gwynenn Taylor Jones, GTJ Dance, Black Dirt Dance, and their respective owners, employees, instructors, agents, contractors, and representatives from claims arising from the ordinary and inherent risks of my voluntary participation. This release does not apply to gross negligence, reckless conduct, intentional misconduct, or any liability that cannot lawfully be waived. 7. Emergency Assistance: If I experience an injury or medical emergency and I am unable to communicate, I authorize the instructor or studio personnel to contact emergency services and my designated emergency contact. I understand that I am responsible for medical expenses arising from my participation or emergency treatment, except where otherwise required by law. 8. Class Policies: I acknowledge that I have reviewed and agree to the published policies concerning: Registration and payment Cancellations and refunds; Missed classes; Package expiration or transfer; Class cancellation; Appropriate conduct and studio safety 9. Severability: If any portion of this agreement is found to be invalid or unenforceable, the remaining portions will continue in effect to the fullest extent permitted by law. 10. Acknowledgment and Electronic Signature: By signing below, I confirm that: I am at least 18 years old. I have read and understood this agreement. I have had the opportunity to ask questions. I voluntarily agree to its terms. I understand that my electronic signature has the same effect as a handwritten signature.*

I have read, understand, and voluntarily agree to the Adult Dance Class Participation Agreement, Assumption of Risk, and Release.

Photo and Video Permission:*

I give permission for photographs or video recordings of me taken during class to be used on GTJ Dance’s website, social media, email communications, and promotional materials.

I do not give permission for photographs or video recordings of me to be used. Declining permission will not affect my ability to participate.
